Here's how to win:
- Pick up some Care to Share referral cards or download the printable form online. - Hand them out to friends and family. - If the patient(s) you refer come in for a consult, you will automatically be entered into the drawing. - The winner will be chosen by a drawing on April 1, 2012.
Referral cards can be picked up at the office, but are not required for entry. Contest ends on March 31, 2012.
